Description
This course is designed for managers, supervisors, health and safety personnel and other individuals who may be involved in reporting, managing or investigating workplace incidents and require an understanding of the principles and processes used to conduct an effective incident investigation. The course introduces accidents, incidents and near misses and considers why workplace incidents should be reported and investigated. It explores responsibilities for incident management, the factors used to determine the appropriate level of investigation and relevant reporting requirements under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013 (RIDDOR). Learners consider the incident investigation process, including making the scene safe, preserving evidence, selecting an appropriate investigation team, inspecting the incident scene and gathering relevant information. The course examines sources of evidence including witness statements, photographs, risk assessments, training and competency records, safe working procedures, permits, maintenance and inspection records, Safety Data Sheets (SDS) and manufacturer`s information. The course also explores how to establish a sequence of events, identify primary root and contributory causes and apply techniques such as the 5 Whys to support effective incident analysis. Learners consider how corrective actions should be developed using the hierarchy of controls, allocated to responsible persons, recorded, implemented and reviewed to help prevent recurrence. The course reinforces the importance of accurate investigation records, appropriate confidentiality, timely reporting and retaining relevant evidence. It also considers statutory RIDDOR reporting timescales and the importance of ensuring investigation findings and corrective actions are appropriately documented and monitored.
